2014 AHMA Conference
Travel Scholarship Application

The second annual patient conference of the American Headache and Migraine Association will be held in Los Angeles, California, on Sunday, June 29, 2014, from 8:00 a.m. to 6:00 p.m.

The AHMA realizes that attending this conference is not financially within reach of all of our members. We are, therefore, offering a number of travel scholarships of up to $500 each to help cover transportation, conference hotel, and meals.


About the scholarships:

  • There are people who absolutely cannot attend without financial assistance. Please do not apply for a scholarship if it is possible for you to attend without this assistance.
  • AHMA board members are not eligible for these scholarships.
  • Each scholarship is intended for one person. It does not include expenses for a companion.
  • Scholarships can be applied to hotel costs for the night before the conference and the night of the conference only.
  • Scholarship recipients are expected to travel to and from the conference by the least expensive mode of travel unless there is a confirmed medical reason to travel by a more expensive mode. In such cases, a doctor's letter stating a medical need to travel by the more expensive mode must be submitted at the same time as the application form is submitted. These letters can be emailed to conference@ahma.ws.
  • Preference will be given to scholarship applicants who have not received a scholarship to our conference previously.

All scholarship recipients will be required to:

  • complete a W-9 form. Recipients will receive a 1099 form for tax purposes.
  • keep and submit copies of receipts for travel, conference hotel, and meals.
  • submit copies of receipts along with any unused portion of the scholarship funding. (Copies of receipts should be submitted. Recipients should maintain the originals.)

Awardees will be contacted during the first week of June, 2014.
